HQA Parent Questionnaire for Fall 2020

Dear Parents,

Assalamu Alikum
In order to prepare well for the coming academic year which will start in August 5, 2020 inshaa Allah, we would like you to help us answer a few questions. The following questionnaire examines your opinion in three areas. These are

1. Assessment of last quarter online school from March 25 to May 15, 2020
2. Hybrid of onsite and online option for the fall of 2020
3. Continuing with the same online delivery system that we had in the last quarter

Please fill in this attached questionnaire for each child you have in the school. This means that if you have one child, you will fill it one time, but if you have 3 children, you will have to fill it three times. Please finish it before July 8^th to allow us to finalize our plans for the fall 2020
